I just want to create a form and have it submitted via email in the format of the field names and the data entered into those fields each on a seperate line.
Your article, although very thorough on the command definitions, does not offer much in the way of a "how to".

Your example page, when you fillout the form and click on submit, you get an error page. Therefore, the tutorial is not very helpful.

Thought you ought to be aware of this error.
